HOW TO MAKE AN INSURANCE CLAIM

Making an insurance claim may be a hassle for some people. Kurnia has come up with a simple guideline that could help you accelerate your claim process. For claim of loss or damage of Property, Marine Cargo/Hull, Engineering, Liability, Movable Property or Motor Vehicle, please follow the steps below:

Procedure to make a claim

	Report all claim within 5 days since the loss happen to the nearest branch of Kurnia Insurance by mail, phone, fax or email and ask for claim form.
	Submit claim form and all supporting documents to Kurnia Insurance.
	For damage of motor vehicle claim, please bring the vehicle directly to Kurnia workshop partner or other workshop which approved by Kurnia Insurance.
	Investigation will be carry if necessary.
	Kurnia Insurance has right to appoint third party to investigate the loss.

Original Invoices, Packing List and Delivery order

Weight Notes at final destination (Berita serah terima barang) (3 pages):

-	From Shipping Company to EMKL (For Export/import)
-	From EMKL to Land Carrier (for Export/import)
-	From Land Carrier to consignee (Insured)

Claim Letter to carrier (responsible party above) and It's reply

Chronology of incident

Police Loss Report (due to loss of Cargo)

Loss Estimation

Damage Cargo List

Photograph (Due to kind of loss cargo)

Q/C Department Letter, which state that damage cargo could not use for production/sale (for the damage cargo)
